
Aesthetically unappealing veins can occur in endurance athletes. While enlarged, straight veins are considered to be normal, due to low body fat and venous engorgement from increased cardiac output, these veins may be treated for cosmetic improvement.
True varicose veins, not only enlarged, but tortuous, may actually affect performance, as some European studies suggest. Some athletes with prominent varicosities are far more likely to suffer from severe leg cramping due to excessive lactic acid buildup, which acts as an irritant not only to muscles, but vein walls as well. Conversely, others may find that their varicose veins cause no symptoms due to increased muscle tone, which assists in venous return from the lower extremities.
It is important, however, to understand that venous disease is progressive, often leading to phlebitis, permanent skin changes and even ulceration. While exercise can heighten or help symptoms, spider and varicose veins will not disappear without treatment. A consultation including an ultrasound exam is recommended during pregnancy only in the presence of symptomatic (painful, swollen, phlebitic) leg veins. While actual treatment of the veins is not done during pregnancy, typically a woman with symptomatic veins will be supported through to delivery with compression stockings and leg elevation when at rest. After delivery, she can pursue sclerotherapy for spider, reticular and pelvic veins, or if needed, endovenous laser therapy (EVLT) for more serious varicose veins resulting from venous insufficiency. In either situation, treatment for these veins is well-tolerated with no downtime, making it possible for the patient to care for her new baby.

Permanently enlarged pelvic, labial or vulvar veins are the result of dilatation that occurs during pregnancy and remains afterward.
